Job Title: Physical Therapist

Schedule: Monday to Friday, 8:00 AM – 4:15 PM

Contract Duration: 13 Weeks 

Pay rate: $ 43 - $46 per hour on W2

Job Summary:

We are seeking a Physical Therapist I (Contract) to join our multidisciplinary rehabilitation team in an outpatient setting. This entry-level role involves providing direct physical therapy services to a diverse patient population, primarily in an outpatient environment, with the possibility of rotation into inpatient and acute care settings. The therapist will work under the supervision of the Therapy Manager or Professional Practice Manager.

Key Responsibilities:

• Perform comprehensive initial examinations and evaluations of patients.
• Develop and implement individualized treatment plans based on clinical findings and patient goals.
• Conduct re-evaluations and adjust treatment plans accordingly.
• Document all evaluations, progress notes, treatment sessions, and discharge summaries in a timely manner.
• Educate patients, families, and caregivers regarding treatment plans, home exercises, and rehabilitation strategies.
• Recommend durable medical equipment as necessary, and write letters of medical necessity when appropriate.
• Participate in discharge planning and coordinate ongoing physical therapy needs.
• Collaborate with a multidisciplinary team to ensure continuity of care.
• Attend rounds, team meetings, in-services, and assigned clinics.
• Provide guidance and supervision to Rehab Patient Care Associates, students, and volunteers.
• Maintain accurate records of attendance, charges, and statistics.
• Provide backup patient coverage as needed due to staffing levels.

Required Qualifications:

• Education:
  • Bachelor's degree in Physical Therapy required.
  • Doctorate in Physical Therapy (DPT) strongly preferred.
• Licensure/Certification:
  • Current New York State Physical Therapy License
  • Active membership with the American Physical Therapy Association (APTA)
• Experience & Skills:
  • 0–1 year of clinical experience
  • Experience with Epic EMR system and outpatient physical therapy settings required
